Please note: Panorama does not work with third-party or auto insurances. Also, Panorama does not accept non-traditional health insurance plans as the primary insurance coverage for services. This includes Cost Sharing plans, Health Care Sharing Ministries, Short-term Health or Gap Insurance, or Limited or Fixed Benefit Plans.

Important Information
Insurance coverage and network participation can change over time. This list is intended as a general guide and is not a guarantee of benefits, coverage, or payment. We encourage all patients to verify their coverage directly with their insurance carrier before receiving services.
Out-of-Network Insurance Plans
If we are not participating with your insurance plan, you may still choose to receive care with us as a self-pay patient.
At this time, we do not submit claims to out-of-network insurance plans. If your plan offers out-of-network benefits, you may choose to submit a claim directly to your insurance carrier for possible reimbursement. Any reimbursement is determined by your insurance plan and is not guaranteed.
If you need documentation to submit a claim, you may request an itemized statement through MyChart.
Choosing an Insurance Plan
If you are shopping for or selecting a new health insurance plan, please note that our staff cannot recommend plans or verify network participation before coverage becomes active.
To verify whether we participate with a plan, we must have an active policy and the member ID number associated with that coverage. Until a plan is active and member information is available, we are unable to confirm network status, benefits, coverage, referral requirements, or out-of-pocket costs.
When comparing insurance plans, we recommend contacting the insurance carrier directly for the most accurate and up-to-date information.
