If you are our patient and would like to access your records, the easiest way is by accessing them in MyChart. If you would like to request your medical records, you may do so by completing your request with our third party  ShareCare. Please click on the link below to complete your request for medical records. You will be required to provide a valid email address and a government-issued ID.
